Forms & Medical p
At Van Allen Integrative Health & Wellness, we understand that your healthcare needs do not always begin and end in the appointment room.
Mental health and medical concerns can affect your work, education, travel, daily responsibilities, and ability to navigate important life obligations. As part of our concierge approach, we provide thoughtful assistance with clinically appropriate forms, letters, and medical paperwork when needed.
Our goal is to make this process as seamless as possible while ensuring that every document we complete is individualized, accurate, and supported by your clinical evaluation and medical record.
Our Commitment to You
At Van Allen Integrative Health & Wellness, we believe exceptional care extends beyond the appointment itself. Our concierge model allows us to provide thoughtful, individualized support when life requires documentation in addition to clinical care.
We take the time to understand your circumstances and ensure that any forms or letters we complete accurately reflect your clinical needs and treatment. We do not believe in generic, one-size-fits-all documentation.
Our commitment is to provide care that is personal, attentive, and designed to support you; not only in your treatment, but in navigating the responsibilities and realities of everyday life.
Covered forms & paperwrok
When clinically appropriate, our providers may assist with:
We may provide documentation supporting medically necessary leave, including appropriate FMLA certification and related medical leave documentation. Because these forms require an accurate understanding of your diagnosis, symptoms, functional limitations, and treatment needs, we generally complete documentation after your initial comprehensive evaluation and sufficient follow-up care to support the request.
When appropriate, we may provide documentation describing medically supported functional limitations and recommendations related to workplace accommodations, such as modified work schedules, flexible scheduling, remote or hybrid work considerations, temporary workload modifications, and additional breaks.
When clinically appropriate, we may provide documentation supporting academic accommodations or medically necessary considerations for high school, college, or university students, including attendance considerations, testing accommodations, temporary modifications, and leaves of absence.
When clinically appropriate, our providers may complete sports participation, pre-participation, or medical clearance forms based on a comprehensive health history evaluation, current symptoms, medications, and relevant clinical considerations.
Travel can sometimes present unique challenges for individuals managing medical or mental health concerns. When clinically appropriate, we may provide medical documentation to support necessary travel or transportation-related accommodations.
With your appropriate authorization, we may provide relevant clinical documentation to support coordination with other members of your healthcare team, including primary care providers, therapists, and specialists for a connected approach to your care.
Process & Expectations
Medical forms and paperwork must be based on an appropriate clinical evaluation and supported by your medical record. For this reason, the timing of paperwork depends on the type and complexity of the request, your clinical circumstances, and the information required to provide accurate documentation.
Some simple letters may be completed following your initial evaluation when clinically supported. More comprehensive documentation, including FMLA certification and workplace accommodation requests, generally requires an established clinical relationship and appropriate follow-up care.
Additional appointments may be required for complex requests or documentation requiring a detailed assessment of functional limitations.
Your scheduled appointments are designed to focus on your clinical care and treatment. While brief documentation may sometimes be addressed during a scheduled appointment, extensive forms, detailed letters, and comprehensive medical documentation may require additional provider time and may be subject to an administrative fee.
We will communicate any applicable fees before completing extensive paperwork.
